Fall Supernova League Team Registration

Build and enter your own team of 6-8 players for our Supernova league! Each player will be required to pay an individual fee of $125. This form should only be filled out once per team by the team captain. Before filling out this form, you must be prepared to provide a list of teammates and a parent's email for each player. This league will NOT come with a team t-shirt; teams can choose to coordinate attire on their own.
